檢查
分為病原學診斷和血清學診斷。前者包括從外周血液、乳糜尿、抽出液中查微絲蚴和成蟲；后者為檢測血清中的絲蟲抗體和抗原。
1.病原學檢查
血檢微絲蚴，由于有夜現周期性，取血時間以晚上九時至次日凌晨二時為宜。
（1）夜間采血檢查微絲蚴陽性
（2）成蟲檢查法在尿、鞘膜積液、淋巴液、腹水、乳糜尿查見微絲蚴，在淋巴管、淋巴結內查見成蟲，或在病理組織切片中查見絲蟲斷面。
（3）特異的DNA探針
2.血清學檢查
快速免疫色譜試驗檢測班氏絲蟲抗原陽性或酶聯免疫檢測絲蟲特異性抗體IgG陽性。
診斷
符合流行病學史，具有典型的臨床表現，同時血清學或病原學檢查陽性即可診斷。
治療
1.病原治療
（1）治療藥物主要是海群生（又名乙胺嗪）。海群生對兩種絲蟲均有殺滅作用，對馬來絲蟲的療效優于班氏絲蟲，對微絲蚴的作用優于成蟲。國內海群生的常用療法為7日療法治療班氏絲蟲病；4日療法治療馬來絲蟲病。患者服藥后可因大量微絲蚴的死亡而引起反應，出現發熱、寒戰、頭痛等癥狀，應及時處理。為了減少海群生的副作用，現在防治工作中廣泛采用了海群生藥鹽，食用半年，可使中、低度流行區的微絲蚴陽性率至1%以下，且副作用輕微。
（2）阿苯達唑，每日兩次，可殺死成蟲，但對微絲蚴無直接作用。
（3）近年我國研制成功抗絲蟲新藥呋喃嘧酮，對微絲蚴與成蟲均有殺滅作用，對兩種絲蟲均有良好效果。對班氏絲蟲病的療效優于海群生。
2.對癥治療
對急性淋巴結炎，受累部位給予局部護理，如足部護理，清洗感染部位，及時給予抗菌藥物治療，足部每天涂抹抗真菌藥膏。對象皮腫患者除給予海群生殺蟲外，還可結合中醫中藥及桑葉注射液加綁扎療法或烘綁療法。對囊象皮腫及鞘膜積液患者，可用鞘膜翻轉術外科手術治療。對乳糜尿患者，輕者經休息可自愈；也可用1%硝酸銀腎盂沖洗治療。嚴重者以顯微外科手術作淋巴管－血管吻合術治療，可取得較好療效。

treatment
Pathogen therapy

2. symptomatic treatment
For acute lymphadenitis, the affected parts of the local care, such as foot care, cleaning the site of infection, timely treatment of antibiotics, foot daily smear antifungal ointment.
